Private/Semi Private Swim Lessons 2025

Required for our report card management software.

Are there any allergies, medical concerns, learning accommodations, or other accommodations the program instructor should be made aware of. If None, please type “None”

If the swimmer completed swim lessons at Selkirk Park Pool in Summer 2024, please include their report card participant number below.
Ex: CoBangert2018 – If the swimmer does not have a report card participant number, please type in “None”.

Please enter an emergency contact that can be called in the event of an emergency. Please include their name, relationship to the participant and phone number. (Ex: Jane Doe, Grandmother, 204-111-1111)

By clicking this you agree to the Program Registration Agreement, along with all program terms and conditions listed on our website. We require a parent/guardian to be presented in the facility for swim lessons for all swimmers under the age of 12.

I give permission to the City of Selkirk to take and use photos or videos of me (or my child, if under 18) for promotional purposes including social media, website, and other communications. I understand these may be used without compensation and may be seen by the public.

We promise not the spam you! We send program and event information and important dates our approximately 4 times per year.
